DO NOT PUT RESIDENTS FROM DIFFRENT COUNTIES ON THE SAME SHEET FOR THEY WILL BE DISQUALIFIED.

PLEASE START A NEW SHEET FOR EACH DIFFERENT COUNTY YOU ARE GATHERING SIGNATURES IN.

NOTARY PUBLIC ARE FORBID FROM CIRCULATING OR SIGNING A CANDIDATE PETITION AS A PETITIONER.
